If you wish to deregister as a patient at our practice, you must do so in writing. This is because of the AVG privacy law.

You can send this instruction of deregistration by e-mail to us: practical@mshimbula.com.

The following information must be included in the deregistration order:

1. Personal information such as your name and date of birth
2. Details of your new dentist (name, address and e-mail address of the new dentist)

As soon as we have received your deregistration order, we will securely send your file to your new dentist. When we have processed the deregistration, you will receive a confirmation by e-mail.
